The HFEA is about to begin the process of compiling the data for the next Patients' Guide and Annual Report. In order to ensure that the information is accurate each clinic will be asked to verify the treatment and outcome data the Authority holds for it on its register for the period 01/04/95 to 31/03/96.

A summary of the IVF and/or DI treatments for your clinic for the same period will be sent to you by 28 March for you to verify against your own records. For IVF this will include all frozen embryo replacements, abandoned cycles and cycles involving micromanipulation. A full list of all treatment cycles will be available upon request. If you require such a list please contact the Joanna Thompson before 4 April.

The deadline for submitting additional forms or making any amendments to the data held on the Register is 9th May. After that date no further data submissions for the period will be included in either the Patients' Guide or Annual Report.
