F-2012-00106 – Incidents relating to vitrification reported to the HFEA since 2003

Summary of request
The Authority was asked for the number of incidents reported to the HFEA since 2003 that related to embryos lost during vitrification. Also, whether any such incident involved the McGill Cryoleaf.
HFEA response
1. A search of our records shows five incidents reported between 2003 and 06 June 2012 involved the loss of embryos during the process of vitrification. As requested, this excludes any in which the loss was stated to be due to embryos failing to survive the thawing process.
2. The HFEA does not routinely require centres to report equipment details and of the incidents identified above none referenced the McGill Cryoleaf.
