Summary of request
The Authority was asked for information about the use of payment cards issued to staff at the HFEA.
1. A list of every transaction made using payment cards where the transaction date occurred during the financial years 2009-10 and 2010-11. For each transaction, please include the transaction date, vendor name, transaction amount and any category recorded by either the payment card issuer or your organisation.
2. Any policies and procedures relating to the proper use and misuse of payment cards and monitoring of their use.
3. All details relating to all instances of misuse of payment cards during the financial years 2009-10 and 2010-11. For each instance, please include: a. transaction date, b. vendor name, c. transaction amount, d. item purchased, e. reason the transaction was not a proper use of the payment card, f. disciplinary action taken, g. if your organisation required the amount to be repaid, and h. if the amount was repaid.

Information in response to each part of the request is set out below and in the attached files.
1. The attached files contain a list of every transaction made using government procurement cards where the transaction date occurred during the financial years 2009-10 and 2010-11. Please note that this is the only type of payment card we have.

2. The HFEA holds four government procurement cards which have been issued to three Directors and one Administrator who is a senior member of staff. At the time of issue, cardholders were advised that spending on these cards would have to be in accordance with our financial procedures, cardholders would need to complete a transaction summary sheet which would then be signed off by their line manager and any supporting documentation would need to be retained for full reconciliation with the credit card statements.
3. There have been no instances of misuse of payment cards during 2009-10 and 2010-11.
