Summary of request
The Authority was asked for ‘relevant dates when inspection notebooks began to be kept or when the relevant records ceased to be kept, as well as the reasons for commencement and cessation of any such practices’.
HFEA response
A new format for the inspection report and the introduction of an inspector’s notebook was presented to the HFEA Regulation Committee on 1 March 2006. The aim of this new documentation was to focus inspections and standardise the assessment of compliance through five topics - organisation and management, quality of service, premises and equipment, information and finally, laboratory and clinical practice.
The Regulation Committee approved the new way of working which was adopted by the inspection team in April 2006. The notebooks remain in use at the present date.
