Updated consent forms and Directions
Further to my letter of 9 July 2009, I am writing to inform you that two new General Directions have been finalised. Direction 0008 refers to information to be submitted to the HFEA as part of the licensing process and Direction 0012 refers to the retention of records. Both Directions will come into force on 1 October 2009, alongside the other Directions published in July.
I would also like to take this opportunity to inform you of some minor amendments and corrections of typographical errors we have made in response to your feedback to the revised Directions and consent forms:
| Directions or consent form | Amendment |
| Directions 0006: Import and export of gametes and embryos | The Directions now require notifications to be submitted to the Authority using the Electronic Data Interchange system |
| ED form | Typographical error corrected: ‘eggs’ replaced with ‘embryos’ at 2.2 |
| CD form | Typographical error corrected: ‘direction’ replaced with ‘direct’ at 4.2 |
| ES and LGS forms | The tick box for consent to storage ‘For 55 years’ has been deleted because these forms should be used for consent to the extended period of storage (not the total period) |
| WD and MD forms | A question has been added to allow donors to consent to the use of embryos (created with their sperm or eggs) in the treatment of others after their death or after they become mentally incapacitated |
