Review of Deceased fathers Act

 

As of 18 September 2003, any man wishing to be recorded as the father of a child resulting from fertility treatment undertaken after his death must have given his written consent. The HFEA produced a sample consent form to fulfil the requirements of the new Act. It can be downloaded below.

In order to ensure that the requirements for registration may be met, the form has two parts. The first part records the man's consent for his particulars to be entered into the birth register, which is required for the purposes of registration if he is alive after 18 September 2003.

The second part records the man's consent to disclosure of information about his treatment to his wife/partner and to the relevant Registrar General for the purpose of registration.

Signed forms should be kept in the patients' file.
