New forms from 1 October 2009

The EU Tissues and Cells Directive and HFE Act 1990 (as amended) mean that there are a few changes required to the data submitted by your centre to the HFEA register.  This will include the introduction of new register data forms and minor amendments to the current patient and partner forms.

Three new forms are required:
 

What the forms are for:

Embryo and Gamete Movement – In       
Records all embryos and gametes coming into storage at your centre.

Embryo and Gamete Movement – Out
Records all embryos and gametes removed from storage at your centre for reasons other than treatment.

Donor Information form
For donors to complete when registering to become a donor.

Donor Sperm Procurement form
Records the storage of donor sperm.

Patient and Partner Registration form guidance note

Welfare of the Child form
A form to determine your patients suitability as parents.

The new embryo and gamete movement forms will replace all the current ‘Notification of imports/exports of gametes and embryos’ forms that are currently sent to the HFEA and greatly reduce the amount of information to be submitted.

The movement forms will also need to be submitted for transfers within the UK and when gametes or embryos are removed from storage for reasons other than treatment (e.g. destruction or donation to research). 

The donor sperm procurement form is a new requirement.
