Updated consent forms
The HFEA consent forms are for the use of patients, donors and their partners at HFEA licenced centres to record consent to various aspects of fertility treatment.

The Human Fertilisation and Embryology Act 2008 made significant changes to the existing legislation regarding consent.
These changes have been reflected in new HFEA consent forms for:
- treatment
- storage
- donation
- disclosure of information, and
- parenthood.
When did it change?
The new consent forms came into force on 1 October 2009.
The forms are more user-friendly and easy to understand. We have developed a guidance document to accompany the consent forms.
