Dear Colleague

Treatment Data for April 1995 to April 1996 on the HFEA Register

Following my letter of 14 March I am writing to enclose a summary of treatment and outcome data for the period 01/04/95 to 31/03/96 in respect of your clinic and I would be grateful if you would verify them against your own records.

In addition to verifying your records against the summary supplied, I would be grateful if you would identify in particular:

• any IVF cycles which were abandoned due to failure of frozen embryos to thaw; and
• any DI cycles which report GIFT with donor gametes.

The printouts also list all treatments that led to a clinical pregnancy together with the outcome of that pregnancy. There are some cases where no outcome form has yet been received. We would be grateful if you could investigate the outcome of these pregnancies and submit a completed outcome form. The Authority appreciates that, despite a clinic's best endeavours, some patients lose contact with the clinic. In such cases please return the outcome form with the outcome of the pregnancy stated as "Lost to follow-up".

You should be aware that the printouts exclude forms received after 20/03/97 or any on which the Authority has a query outstanding with the clinic.

The deadline for submitting additional forms or making any amendments to the data held on the Register is 9th May. After that date no further data submissions for the period will be included in either this year's Patients' Guide or Annual Report. If you have any amendments to the data enclosed, or any other related queries, please contact Joanna Thompson at the HFEA.
