Dear Colleague,

Audit of Stored Sperm, Eggs and Embryos

A recent Chairman's Letter CH(00)10 asked all centres to conduct an audit of all material in storage. This letter is being circulated to centres specifically to clarify what information should be submitted to the HFEA.

All centres should supply the following information to the HFEA by 15th December 2000:

details of the procedures and protocols used to conduct the audit;
the date(s) the audit was carried out;
the specific findings of the  audit i.e. whether any discrepancies were discovered and whether all consents were correct and valid.
Please do not send in any patient names.

This report should be signed and dated by the Person Responsible.
