Dear Colleague,

Request for information regarding media used in IVF

I notice that we have not received a response to my letter of 26th June 2001 (Ref: CE(01)02) in which I requested information on the media in use at your centre. In case you did not receive this letter, a second copy is enclosed. We wish to assemble as complete a picture as possible of the situation regarding media in the UK, and your input into our risk assessment exercise would be greatly appreciated.

I appreciate that we are asking centres to undertake a considerable amount of work, particularly in relation to the historical information requested, however information on the current situation should be readily available within the clinic.
