HFEA Donor information forms

As you will be aware, new Regulations relating to the disclosure by the HFEA of information about gamete and embryo donors come into force on July 1. These Regulations are the Human Fertilisation and Embryology Authority (Disclosure of Information) Regulations 2004 (S.I. 2004 No.1511).

The Regulations require the HFEA to disclose to applicants (donor-conceived people over the age of 18) certain items of non-identifying information regarding the donors used in their conception.  Furthermore, for those conceived using donors who register after April 1, 2005 (including past donors who re-register after that date), the regulations also require the HFEA to provide identifying information about those donors to applicants who request it.

In order to ensure that the HFEA collects the information needed to discharge this function in the interests of donor-conceived people new donor information forms have been produced.  Copies of the new green form ? form D (version 2004) ? together with Directions bringing the forms into force are enclosed.  The forms should be completed in accordance with the guidance contained in "Donor Information Form Version 2004 ? Guidance Notes on completing the HFEA forms" (also enclosed).  A copy of this guidance may also be found on the HFEA Centres' website at: https://centres.hfea.gov.uk/HFEAPublications/HFEAForms

Forms other than the donor information form should continue to be used in accordance with Direction D. 1999/1.  Please note that the enclosed Directions require centres to begin using the new forms for all donors registering on or after July 1, 2004.  You should therefore continue to use the old (green) form - form HFEA Register (99) D.I 0. - up to that date.  On July 1, you should destroy any unused (99) D.I.0. paper forms and begin using the new form.
