What the forms are for:

As a result of the EU Tissues and Cell Directive, the HFEA is required to collect summary statistics of activities for IUI and GIFT treatments using partner sperm on an annual basis. Please submit your annual return for IUI & GIFT with partner sperm via the clinic portal: HFEA clinic portal
