This form should be completed and sent to the HFEA once confirmation of the early outcome of a treatment is known (e.g. foetal heart seen, biochemical pregnancy only, miscarraige, not pregnant). On the form, the number of gestational sacs seen is required if a positive fetal heart is seen. |
