What the forms are for:

This form should be completed and sent to the HFEA once confirmation of the early outcome of a treatment is known (e.g. foetal heart seen, biochemical pregnancy only, miscarraige, not pregnant).

On the form, the number of gestational sacs seen is required if a positive fetal heart is seen.

What the forms are for: This form is to be sent to the HFEA after fertility treatment results in a pregnancy. The form also records whether a congenital abnormality is observed.
