Person Responsible Entry Programme

An individual can be appointed as the PR only with the approval of the HFEA. That person must complete the Person Responsible Entry Programme (PREP) assessment before the HFEA can consider whether or not to approve them. |

Renewal of licence form
| What the forms are for: |
Use this application form to renew your existing HFEA licence for treatment and/or storage of gametes and embryos. From 6 April 2010 all treatment or research licence applications will have to be submitted directly to the HFEA via the clinic portal. |
Variation to your clinic’s licence forms
| What the forms are for: | You must fill out a relevant variation form if you are planning on making a change at your clinic that alters the conditions of your original licence. |

Pre-implantation genetic diagnosis (PGD) application forms

Apply for authorisation to carry out PGD for new conditions (that have not been previously authorised by the Authority) and for case-by-case genetic disorders or to tissue type embryos. |
